  • Full-time We are seeking a compassionate, experienced, and dedicated Physician Assistant/ Nurse Practitioner (NP) for a Nursing Home position to provide comprehensive care for patients, including diagnosing, treating acute/chronic conditions, ordering tests, managing care plans, and ensuring smooth transitions of care
Key Responsibilities:
Clinical Assessment & Diagnosis:
Conduct comprehensive physical examinations and health histories for residents. Order, interpret, and evaluate diagnostic tests and results (lab work, imaging studies) to diagnose acute and chronic health conditions.
Treatment Planning & Management:
Develop, implement, and manage individualized treatment plans, including prescribing medications and other therapeutic interventions such as physical or occupational therapy.
Care Coordination:
Collaborate effectively with physicians, nurses, social workers, pharmacists, and other healthcare professionals to ensure continuity of care and holistic resident management.
Patient & Family Education:
Educate residents and their families on disease processes, medication management, self-care strategies, and health promotion practices to encourage informed decision-making and optimal health outcomes.
Documentation & Compliance:
Maintain accurate, complete, and confidential electronic health records (EHRs), ensuring all documentation is compliant with facility policies and regulatory guidelines.
AI Integration & Data Use:
Utilize AI-powered tools such as clinical decision support systems (CDSS) and predictive analytics to identify potential health risks (e.g., fall prevention, early detection of deterioration), streamline administrative tasks, and enhance diagnostic accuracy.
Quality Improvement:
Participate in quality improvement initiatives and utilize data-driven insights from AI tools to enhance clinical practice, optimize resource allocation, and improve overall resident care quality.
Qualifications & Skills Education:
Master's degree in Nursing (MSN) from an accredited program; Nurse Practitioner certification (e.g., FNP, AGNP).
Licensure:
Current, active, and unencumbered state Nurse Practitioner license and national certification.
Experience:
Proven experience in a clinical setting, preferably in geriatrics, long-term care, or a nursing home environment.
Clinical Skills:
Strong clinical judgment, diagnostic, and treatment skills, with the ability to work autonomously.
Technical Proficiency:
Familiarity with Electronic Health Records (EHRs) and an understanding of AI applications in healthcare for documentation and decision support.
Soft Skills:
Excellent communication, interpersonal, and leadership skills, with a compassionate and empathetic approach to patient care.
Adaptability:
Eagerness to adapt to technological advancements and integrate AI tools effectively into daily workflows.
