Qualifications Current patient population served: 18 years + Applicants must have a current license to practice in Texas Certified Nurse Practitioner or Physician Assistant Full Job Description General Overview ADIRA Health is seeking a Physician Assistant or Family Nurse Practitioner (PA/FNP-C) to join our growing primary care team in a hybrid position serving both our Mission and Pharr, Texas locations. Position Summary:
The Nurse Practitioner-Physician Assistant is directly responsible for care of patients. Duties include performing initial clinical evaluations as well as directed procedures for urgent care, occupational health, and workers' compensation procedures. The Nurse Practitioner-Physician Assistant will also follow up on laboratory analysis and radiology studies. The job description below is not all inclusive, and will perform other job duties assigned by supervising physician.
Essential Responsibilities :
1. Maintains customer loyalty score at or above company benchmark. 2. Participate in huddles with management and office staff. 3. Consistently communicates with patients on wait times and/or delays; provide direction to the next step in the treatment process 4. Provides solutions to patient problems and recognizes and seeks help in emergent/hazardous situations. 5. Provide timely follow-up regarding lab and x-ray results. 6. See patients in different clinics in the Pharr area along with telemedicine 7. Practice in collaboration with our Contracted and Affiliated physicians, their advance practice clinicians, and their clinic staff. 8. Collaborate to provide a comprehensive, coordinated model of care to a mostly Medicare-aged population on a Medicare Advantage Plan. 9. Performance will be tracked and comparatively reported using metrics related to the strategic direction and operational needs of the organization. 10. Provides direct care, within their scope of practice, to adult patients to include urgent care. 11. Consults with the Supervising Physician, Physician, or Pediatric Advisor for complex medical, pediatric, or psychiatric patients, completes orders recommended/authorized by the physician. 12. Performs history and physical assessment, behavioral health care, minor outpatient procedures, patient education, and refers any care beyond the scope of the medical unit to local medical facilities emergency departments or urgent care centers. 13. Ability to apply professional principles, procedures, and techniques for Nurse Practitioners in patient care. 14. Knowledge of drugs and therapeutics when prescribing and dispensing medication. 15. Obtains and accurately documents patient medical record and treatment. 16. Ensures that equipment is in proper working order and reports any malfunction to the center manager or designee on duty. 17. Follows documentation procedures and completes required paperwork related to patient visit. 18. Adheres to infection control/safety guidelines and patient confidentiality policies. 19. Act in supervisory role while on duty ensuring policies and procedures are followed, logbooks are maintained, examination rooms are stocked, and other duties as assigned. 20. Act with honesty and integrity in all business transactions, including, but not limited to, employment applications/resumes, patient records, time records, and financial transactions.
Pay:
$90.00 - $100.00 per hour
