Advanced Practice Provider (Family Medicine / Women's Health) Daytime Schedule

  • Townsen Physician Group | Bellaire, TX 77072 Townsen Physician Group is seeking a dedicated Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Bellaire, TX practice.
This role offers the opportunity to provide comprehensive, patient-centered care across the lifespan in a collaborative, supportive clinical environment. Position Summary The Advanced Practice Provider delivers high-quality, evidence-based care focused on preventive health, chronic disease management, and care coordination . Working closely with physicians, specialists, and interdisciplinary team members, this role emphasizes continuity of care, patient education, and improved long-term health outcomes for diverse patient populations. Key Responsibilities Perform and promote preventive screenings (health assessments, physical exams, etc.) in accordance with clinical guidelines Provide women's wellness services, including: Pap smears Breast exams Contraception counseling Menopause management Diagnose and manage acute and chronic conditions using evidence-based clinical judgment Develop, implement, and adjust individualized treatment plans Order and interpret diagnostic tests, labs, and imaging Prescribe medications and treatments in compliance with state/federal regulations and company protocols Educate patients and families on disease prevention, healthy behaviors, and care plan adherence Coordinate care with providers, specialists, and support services to ensure continuity Maintain accurate and timely documentation in the electronic medical record (EMR) Participate in interdisciplinary meetings and quality improvement initiatives Perform other related duties as needed Knowledge, Skills & Abilities Strong clinical assessment, diagnostic, and decision-making skills Solid understanding of chronic disease management, preventive care, and health maintenance Knowledge of current evidence-based guidelines, screening recommendations, and immunization schedules Excellent communication and interpersonal skills with diverse patient populations Ability to adapt quickly to changing priorities or patient conditions Ability to work both independently and collaboratively Proficiency with EMR systems Strong attention to detail and organizational skills Proficiency in Microsoft Office (Outlook, Excel, Word) Compassionate, energetic, and patient-focused approach to care Education & Experience Requirements Nurse Practitioner Active Texas APRN license National certification as a Family Nurse Practitioner (FNP-BC or FNP-C) through ANCC or AANP Prescriptive authority from the Texas Medical Board Current, unrestricted DEA certificate BLS certification PALS certification Minimum 2 years of family or primary care experience Physician Assistant Active Texas PA license National certification as a PA-C through NCCPA Prescriptive authority from the Texas Medical Board Current, unrestricted DEA certificate BLS certification PALS certification Minimum 2 years of family or primary care experience About Townsen Physician Group Townsen Physician Group is committed to delivering compassionate, coordinated, and high-quality care to patients and families throughout the communities we serve.
