Job Description
SUMMARY:
The Advanced Practice Provider is responsible for providing comprehensive, patient-centered care to individuals of all ages across the lifespan. This role focuses on preventive health, chronic disease management, and care coordination to enhance patient outcomes and quality of life. The Advanced Practice Provider collaborates with physicians, specialists, and interdisciplinary team members to ensure seamless, evidence-based and compassionate care is delivered.ESSENTIAL FUNCTIONS
Facilitate, perform, and promote preventive screenings (health assessments, physical exams, etc.) in accordance with clinical guidelines, ensuring timely communication of results and appropriate follow-up care for patients of all ages. Perform women's wellness services, including pap smears, breast exams, contraception counseling, and menopause management. Diagnose and manage acute and chronic conditions, utilizing evidence-based clinical judgment and best practice guidelines. Develop, implement, and adjust individualized treatment plans based on patient needs, goals, and response to therapy. Order and interpret diagnostic tests, laboratory studies, and imaging as appropriate. Prescribe medications and treatments in accordance with state and federal regulations and company protocols. Counsel and educate patients and families on disease prevention, healthy behaviors, and care plan adherence. Coordinate with providers, specialists, and support services to ensure continuity and quality of care. Maintain accurate, timely, and compliant documentation within the electronic medical record (EMR). Participate in interdisciplinary team meetings and contribute to quality improvement initiatives. Perform other related tasks as needed.KNOWLEDGE, SKILLS, AND ABILITIES
Strong clinical assessment, diagnostic, and decision-making skills. Knowledge of chronic disease management, preventive care, and health maintenance. Knowledge of current evidence-based guidelines, screening recommendations, and immunization schedules. Excellent communication, counseling, and interpersonal skills to build trust with diverse patient populations. Ability to respond quickly, physically and mentally when priorities and/or the patient's condition changes. Ability to work independently with little to no supervision and collaboratively with multidisciplinary teams. Flexibility and adaptability in a dynamic clinical environment. Proficiency in electronic medical record (EMR) systems. Exhibit a caring, compassionate, high level of energy, congenial, sensitive attitude toward patients. Strong computer skills with proficiency in Microsoft Office, including Outlook, Excel, Word. Strong attention to detail, being careful about detail and thorough in completing work tasks. Ability to adequately diagnose, plan and implement nursing interventions and evaluate patient condition.EDUCATION AND EXPERIENCE
Nurse Practitioner Advanced Practice Registered Nurse (APRN) license in Texas National certification as a Family Nurse Practitioner (FNP-BC or FNP-C) through ANCC or AANP Prescriptive authority from the Texas Medical Board Current and unrestricted DEA certificate Basic Life Support (BLS) certified Pediatric Advanced Life Support (PALS) certified Two (2) years of family or primary care experience Physician Assistant Physician Assistant license in Texas National Certification as a Physician Assistant (PA-C) through NCCPA Prescriptive authority from the Texas Medical Board Current and unrestricted DEA certificate Basic Life Support (BLS) certified Pediatric Advanced Life Support (PALS) certified Two (2) years of family or primary care experience Premier Medical Resources reserves the rights to change or modify the employee job description whether orally or in writing, at any time during the employment relationship.
