Full Job Description Job Summary:
We are seeking a dedicated and experienced Nurse Practitioner (NP) to provide high-quality, patient-centered care in a post-acute setting. The NP will work collaboratively with physicians, nursing staff, and interdisciplinary teams to manage the medical needs of patients transitioning from hospital to skilled nursing, rehabilitation, or long-term care facilities. This role focuses on improving patient outcomes, reducing hospital readmissions, and ensuring continuity of care.
Key Responsibilities:
Perform comprehensive assessments, including history and physical exams for post-acute patients Develop, implement, and manage individualized care plans Diagnose and treat acute and chronic conditions Order, interpret, and evaluate diagnostic tests and labs Collaborate with physicians, nurses, therapists, and facility staff to coordinate care Provide timely follow-ups and manage transitions of care Educate patients and families on treatment plans, medications, and preventive care Ensure accurate and timely documentation in compliance with regulatory standards Participate in quality improvement initiatives aimed at reducing readmissions and improving outcomes
Qualifications:
Current, active Nurse Practitioner license in Texas Board certification as a Nurse Practitioner (e.g., FNP, AGNP) DEA license and prescriptive authority Minimum [1-2+] years of experience in post-acute care, geriatrics, internal medicine, or related field preferred Strong clinical decision-making and diagnostic skills Excellent communication and interpersonal skills Ability to work independently and collaboratively in a fast-paced environment
Preferred Skills:
Experience in skilled nursing facilities (SNF), long-term care, or rehabilitation settings Familiarity with EMR systems Knowledge of CMS guidelines and post-acute care regulations
