About Two Rivers Community Health Two Rivers Community Health is a Federally Qualified Health Center serving Green River and Moab, Utah. About the Position We are seeking a Physician Assistant or Nurse Practitioner to help establish and grow primary care services at our new Moab location. This position will ultimately be based primarily in Moab. During the startup period, the provider should expect regular travel to our established Green River clinic for onboarding, patient care, training, team collaboration, and development of workflows for the new site. As the Moab clinic becomes established, the position will transition to being Moab-based, with occasional travel to Green River as needed. Our providers care for patients across the lifespan and provide comprehensive primary care, including assessment, diagnosis, treatment, preventive care, chronic disease management, and care coordination. Responsibilities Provide primary care to patients across the lifespan Conduct histories and physical examinations Diagnose and treat acute and chronic conditions Develop and manage patient treatment plans Prescribe medications within applicable scope of practice Order and interpret diagnostic testing Perform common outpatient procedures, including injections, suturing, and wound care Provide preventive care, patient education, and counseling Coordinate referrals and specialty care Maintain timely and accurate documentation in the EHR Collaborate with other providers, Mental Health, dental, and the broader care team Help develop clinical workflows and services at the new Moab location Qualifications Current, unrestricted Utah license as a Physician Assistant or Nurse Practitioner Appropriate national certification for the applicant's profession Eligible for Medicare and Medicaid credentialing DEA registration or ability to obtain, as required for the position Current BLS certification Comfortable providing care across the lifespan Strong communication and collaboration skills Ability to work effectively as part of a multidisciplinary healthcare team Preferred Qualifications One or more years of primary care or similar clinical experience Experience in rural or community health Bilingual English/Spanish

Compensation & Benefits Salary:
$115,000-$135,000 per year, depending on experience and qualifications.
