Nurse Practitioner Ascend Medical - 4.0 North Salt Lake, UT Job Details 7 hours ago Qualifications BLS Certification Nurse Practitioner (NP) DEA License Advanced practice provider Full Job Description Must have license in hand! Work in clinic to see patients for physicals, women's health, chronic conditions (hypertension, diabetes) M-F 8a-5p with flexible coverage for needs for possible Saturdays. No call.

Clinic-Outpatient PPD/Census:
20-22 patients per day Full support staff-
RNs, MAs, LVNs EMR:
Epic CV (with all gaps of 30+ days explained on the CV). Work history and education must be in MM/YYYY format. Copy of a clean sex offender search ( https://www.nsopw.gov) COVID Vaccine Documentation/Exemption letter Active License Verification (showing EXP date) Active Board Certification Verification (showing current) Certifications required/accepted: Board Certified, BLS (AHA), DEA (Both OR and WA) Must have active OR license and obtain a WA license
