PRN Nurse Practitioner - ACO Programs Community Nursing Services (CNS) | St. George, UT | PRN / Flexible Schedule Looking to earn more outside your primary care hours? This one's for you. If you're an NP with time between your regular shifts or clinic schedule, this PRN opportunity lets you put your clinical skills to work in a meaningful, flexible way — on your schedule, at $65-$75 per visit. You'd be conducting in-home comprehensive health evaluations for ACO members, ensuring their chronic conditions are accurately documented, care gaps are closed, and their Risk Adjustment Factor (RAF) score truly reflects the complexity of their health — so the ACO receives appropriate reimbursement and patients get the level of care they actually need. Think of it as making sure no one falls through the cracks. Compensation $65-$75 per visit (per-visit rate, not hourly) we will also provide an hourly rate for non-clinical work as well. $0.57/mile mileage reimbursement Flexible PRN schedule — ideally 8-10 visits/week, including some virtual visits What You'll Do Perform comprehensive in-home physical assessments, medication reviews, and chronic disease evaluations Identify and document HCC coding gaps — conditions that may be present but undocumented — to ensure accurate risk adjustment and appropriate ACO reimbursement Assess social determinants of health and home safety risks Complete a visit summary returned directly to the patient's primary care provider to support continuity of care Coordinate with PCPs and care teams to close quality measure gaps (HEDIS, STARS) Educate patients on disease management, preventive care, and self-management What You'll Need Active Utah NP license (FNP, AGNP, or similar) in good standing National board certification (ANCC or AANP) DEA/controlled substance prescribing authority Experience in chronic disease management, geriatrics, or primary care preferred Valid driver's license, reliable transportation, and current auto insurance Ability to pass background check and drug screen Why CNS?
