Job Description

Role Overview Avail Health is seeking a part-time Mobile Nurse Practitioner to provide comprehensive, patient-centered care to Medicare Advantage members residing in skilled nursing facilities in Baker and Franklin, West Virginia. This role combines in-person facility visits with virtual care and care management activities, supporting medically complex Medicare beneficiaries living in long-term care settings. The Nurse Practitioner works collaboratively with an interdisciplinary team of nurses, care coordinators, facility staff, and other healthcare professionals to deliver proactive clinical care, reduce avoidable hospitalizations, and improve quality of life for a frail senior population. This is an ideal opportunity for an experienced Nurse Practitioner who enjoys practicing autonomously, building relationships with facility staff, and caring for older adults with complex medical needs. What You'll Own Management of an assigned panel of Medicare Advantage members residing in skilled nursing facilities Comprehensive primary, acute, and transitional care services Individualized care planning and ongoing clinical management Hospitalization prevention and transitions of care support Collaboration with facility partners and interdisciplinary care teams Quality, documentation, and regulatory compliance Contribution to program growth and continuous improvement initiatives What You'll Do Deliver comprehensive primary and acute care services to Medicare Advantage members residing in skilled nursing facilities Conduct initial evaluations, routine follow-up visits, and transition of care assessments Assess, diagnose, and manage acute illnesses, chronic conditions, and geriatric syndromes Develop, implement, and adjust individualized care plans Order, interpret, and act on laboratory and diagnostic studies Prescribe medications and therapeutic interventions Provide proactive follow-up to monitor clinical status and prevent avoidable hospitalizations Manage admissions, readmissions, and discharges Collaborate closely with facility staff and interdisciplinary teams Participate in interdisciplinary care planning meetings Maintain timely, accurate, and compliant documentation Support Medicare Advantage care management and quality initiatives as assigned What Success Looks Like High-quality, proactive care delivered to a medically complex senior population Reduced avoidable hospitalizations and emergency department utilization Consistent completion of required visits and care management activities Strong collaboration with facility partners and interdisciplinary teams Accurate and timely documentation supporting quality and compliance goals Meaningful contributions to quality metrics and program growth What You Bring Required MSN or DNP from an accredited program Current FNP or AGNP certification Active, unrestricted West Virginia NP license with
WV CARES
approval Active DEA registration with prescriptive authority in West Virginia Licensure in Ohio, Indiana, and Maryland, or willingness to obtain after hire Minimum two years of Nurse Practitioner experience Strong clinical assessment, communication, and documentation skills Comfort practicing independently while collaborating with remote teams Ability and willingness to travel regularly to facilities in Baker and Franklin, West Virginia Preferred Experience in geriatrics, long-term care, mobile care, home-based care, or value-based care models Experience caring for medically complex or frail elderly populations Familiarity with Medicare Advantage, HEDIS, STAR measures, risk adjustment, and transitions of care Experience working within interdisciplinary care teams
Schedule and Work Style Work Type:
Part-time W-2
Location:
Candidates should be located within reasonable driving distance of Baker and Franklin, West Virginia. Harrisonburg, Virginia and surrounding areas are preferred for recruiting purposes but not required.
Schedule:
Approximately four in-person facility days per month, with additional virtual care and care management activities based on patient needs and provider availability
Travel:
Regular travel to assigned skilled nursing facilities required
Autonomy:
High. Functions independently in the field while collaborating closely with Avail's remote clinical and operational teams.
Compensation and Perks Hourly Rate:
