Southern Virginia Job Title:

PA Schedule:
Full-Time (40 hrs/wk) | Mon-Fri, 8:00 AM•5:00 PM (No weekends, holidays, or on-call)

Travel during shift between 2 facilities in
Carroll County VA Pay and Benefits:
$64•$68 per hour, Weekly pay, Health, Dental & Vision Insurance & 401K. Role Summary Join the Senior Community Care (SCC) program delivering integrated, longitudinal care to high-risk health plan members in long-term care settings. Works as part of an interdisciplinary team offering flexible scheduling and strong work-life balance. Key Responsibilities Conduct comprehensive assessments and manage complex acute/chronic medical and behavioral conditions. Conduct advance care planning discussions and educate patients/caregivers. Travel between 2 designated facilities during shift hours.
Required Qualifications Licensure:
Active, unrestricted VA NP or PA license &
DEA Physical & Requirements:
Reliable transportation; ability to lift 30 lbs and navigate stairs/dwelling conditions; full
COVID-19
