Job Description

Lively Medical Center is seeking a Family Nurse Practitioner (FNP) to join our established outpatient primary care team. This is a full-time, outpatient-only position offering a consistent schedule and strong clinical support.

The ideal candidate is committed to delivering comprehensive primary care across the lifespan and values working within a collaborative, team-based model.
Position Highlights:
Outpatient-only practice
Schedule:
7:00 AM - 5:00 PM, 4 days per week.
Patient Volume:
18-22 patients per day
Call:
Rotating call schedule shared among 10+ providers Supported by dedicated clinical staff (CMA/LPN) for rooming and patient flow Provide comprehensive primary care services including: Routine wellness visits Medicare Annual Wellness Visits Acute care visits Hospital follow-ups Preoperative examinations Chronic disease management
Qualifications:
Current certification as a Family Nurse Practitioner Active and unrestricted state licensure DEA certification (or eligible) Commitment to high-quality, patient-centered care Strong communication and teamwork skills
