Encompass Health Mechanicsburg is seeking a dedicated Case Manager to join our team committed to providing high-quality, integrated care. The Case Manager will be instrumental in coordinating comprehensive care arrangements for patients recovering from life-changing illnesses and injuries. This role requires an individual who can effectively communicate and collaborate with the medical team, patients, and their families to ensure optimal healthcare outcomes and a successful transition back to daily life. Responsibilities Develop and implement individual patient care plans in collaboration with healthcare professionals. Facilitate and coordinate resources necessary for patient treatment and recovery. Regularly evaluate and document patient progress and adjust care plans accordingly. Ensure compliance with healthcare regulations and maintain the utmost confidentiality of all patient records. Educate patients and families about treatment plans, including follow-up care and facilities. Required Skills Strong organizational and communication skills. Ability to multitask and manage multiple cases simultaneously. Proficiency in using medical software for case management. Knowledge of healthcare laws, regulations, and standards.
