- Communicate lab and diagnostic test results to patients per provider instructions. Lab Beans

Requirements:
Pt seen withing a year, medication that on the med list Process chronic prescription refills in accordance with provider directives and organizational policies. Support providers with clinical tasks, ensuring efficient workflow and patient-centered care. Collaborate with the clinical access team to facilitate timely patient access to care. Triaging the documents from outside visits to determine any issues that need to be addressed in a timely manner. Assist in completion of documents including conducting patient visits for form completion if necessary. Conduct specific types of patient visits. Maintain accurate and timely documentation in the electronic health record (EHR). Adhere to all clinical protocols, regulatory requirements, and best practices. Participate in quality improvement initiatives to enhance patient outcomes. Nurse Practitioner- Promotes the mission, vision, and values of HealthWorks.
