Overview Join our laid back healthcare team as a Post Acute Care Clinician, where you will play a vital role in coordinating and delivering high-quality post-acute care services. This position offers an exciting opportunity to collaborate with multidisciplinary healthcare professionals, ensuring seamless patient transitions from hospital to home or other care settings. Your expertise will help optimize patient recovery, improve health outcomes, and enhance overall patient experience through comprehensive assessment, planning, and management of post-acute care needs. Duties Conduct patient assessments evaluations to determine post-acute care needs. Develop individualized discharge plans in collaboration with, primary care providers, and community resources to ensure smooth transitions. Facilitate communication between patients, families, healthcare professionals to ensure clarity on treatment goals and follow-up procedures. Monitor patient progress and adjusting care plans as needed to promote recovery and prevent readmission. Ensure compliance with HIPAA regulations and health policy standards while maintaining accurate documentation of all patient interactions and interventions. Skills Family medicine, Internal Medicine, or Geriatric medicine preferred Ability to perform patient assessments and adjust treatment plans Experience working with electronic health records (EHR/EHR management systems) Compassion in caring for elderly and patients with chronic care needs Join us in making a meaningful difference by providing compassionate post-acute care that empowers patients on their journey to recovery!
