Nurse Practitioner - HouseCalls - Franklin, Henry, & Pittsylvania Counties Optum 401(k) United States, Virginia, Rocky Mount Oct 07, 2026

Danville, VA (serving Franklin, Henry & Pittsylvania Counties) | Optum Home & Community Care / UnitedHealth Group Join Optum HouseCalls, an innovative in-home care program focused on preventive health assessments for Medicare Advantage and other health plan members. In this field-based, non-prescribing role, you'll conduct comprehensive annual health evaluations in patients' homes, identify care gaps, provide education, and collaborate with primary care providers to improve outcomes while enjoying significant scheduling flexibility and work-life balance. Position Highlights $10,000 Sign-on Bonus OR $20,000 Student Loan Repayment Bonus available

Flexible provider schedules available between 20-40 hours per week

Flexible scheduling between 8am-8pm across a 7-day work week to support work-life balance

No on-call, weekends, or holidays required

Conduct in-home health assessments and work within an interdisciplinary care model focused on preventive care and care coordination

Required Qualifications

Active ANCC or AANP certification (Family, Adult, Geriatric, or Adult-Gerontology Primary Care NP)

Active, unrestricted NP license in the state of practice (or ability to obtain by start date)

Active, unrestricted driver's license and reliable transportation for home visits

Ability to navigate varied home environments and perform in-home assessments Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
