Overview Join our dynamic healthcare team as a Nurse Practitioner specializing in Long-Term Care (LTC) with a hybrid schedule beginning your day between 7-8:30 a.m. ending your 8 hour shift following shift change at 3:00 p.m. at home at your computer. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and detailed medical histories for residents in LTC facilities. Manage acute and chronic conditions, utilizing skills in critical care, geriatrics, dementia care, and behavioral health to develop personalized treatment plans. Administer medications, including injections and IV infusions, while ensuring proper documentation within EMR/EHR systems such as Epic or eClinicalWorks. Perform specialized procedures such as suturing, catheterization, spinal taps, and dermal fillers when appropriate. Coordinate care through discharge planning, case management, and utilization review to optimize patient outcomes across inpatient and outpatient settings. Engage in telehealth consultations and triage to provide timely medical advice remotely. Maintain compliance with HIPAA regulations and ensure accurate medical records management while adhering to OSHA standards for infection control and sterile processing. Qualifications Valid Nurse Practitioner license with current certification in family practice or geriatrics. Proven experience working in long-term care environments such as nursing homes or assisted living facilities. Strong clinical skills including familiarity with

ICD-10/ICD-9

coding, CPT coding, and medical documentation practices. Experience with EMR/EHR systems like Epic, Cerner, or Athenahealth is highly desirable. Knowledge of complex medical procedures including dialysis management, gastrostomy tube feeding, airway management, and ventilator support preferred. Demonstrated ability to handle acute care situations such as ICU or Level I Trauma Center experience is advantageous. Excellent communication skills with the ability to collaborate effectively within multidisciplinary teams. Additional expertise in areas like hospice & palliative medicine, infectious disease care, occupational health, or behavioral health will be considered a plus. $120,000.00 - $135,000.00 per year
