Title:
Nurse Practitioner (Primary Care & Post-Hospital Follow-Up)
Status:
Part-Time (2-3 Days Per Week)
Location:
Williamsburg, VA (including Williamsburg, James City, New Kent, York, Gloucester, and Mathews) Role Overview The Nurse Practitioner will provide comprehensive primary and transitional care, including post-discharge evaluations, chronic and acute condition management, and ongoing patient monitoring. This role works closely with patients, caregivers, and interdisciplinary healthcare teams to support successful care transitions and help reduce avoidable hospital readmissions. Key Responsibilities Provide primary care and post-hospital follow-up services in patient homes and senior living communities Conduct comprehensive health assessments and follow-up evaluations Complete post-discharge visits and support safe transitions from hospital to home Evaluate, diagnose, and manage acute and chronic medical conditions Develop individualized treatment and care plans Prescribe medications and order appropriate diagnostic tests Coordinate referrals and collaborate with interdisciplinary care teams Educate patients and caregivers regarding medications, disease management, and preventive care Monitor clinical progress and adjust treatment plans when appropriate Complete EMR documentation within 24 hours of each patient encounter Consult with the Medical Director or collaborating providers when clinically necessary Required Qualifications Active Virginia Nurse Practitioner license Current DEA registration Active CAQH profile and CAQH number At least one year of Nurse Practitioner experience in primary care, family medicine, senior care, assisted living, long-term care, or transitional care Experience working with electronic medical record (EMR) systems Strong clinical assessment skills and ability to make independent clinical decisions Preferred Qualifications Previous experience in post-acute or transitional care Background in urgent care, emergency medicine, hospital medicine, or acute care Experience treating geriatric patients and individuals with medically complex conditions Prior experience providing care in patient homes or senior living environments Key Skills Required Primary care assessment and diagnosis Post-hospital and transitional care management Chronic disease management Independent clinical decision-making Care coordination EMR documentation Patient and caregiver education Why This Location? Pay:
$70.00 - $86.00 per hour
