We're looking for a committed Nurse Practitioner or Physician Assistant to join our growing Post-Acute Care team in Bennington, VT, with additional openings in Wakefield, RI and North Conway, NH. This full-time role focuses on caring for patients as they transition from the hospital to skilled nursing and rehab settings, working closely with interdisciplinary teams to manage acute and chronic conditions, support recovery, reduce readmissions, and improve overall outcomes. What the role looks like Facility-based rounding in post-acute care settings, building long-term relationships with patients, families, and on-site care teams Collaborative, team-oriented practice while enjoying clinical autonomy and strong operational support Focus on assessment, diagnosis, treatment planning, medication management, and care coordination to optimize recovery and prevent unnecessary hospital returns Schedule and work-life balance Full-time, Monday-Friday, 9:00 AM-5:00 PM No weekends and no on-call responsibilities Compensation and benefits Base salary: $120,000 annually Total earning potential: $180,000-$220,000+ (depending on productivity and incentives) Comprehensive health, dental, and vision coverage Retirement savings plan Four weeks paid vacation CME support and professional development resources Who we're looking for Graduate of an accredited NP or PA program Current, unrestricted NP or PA license for the state or eligibility to obtain licensure National board certification (NP or PA) DEA registration or eligibility to obtain Minimum of one year clinical experience preferred; background in post-acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care is a strong plus Strong clinical assessment and diagnostic skills, with effective care coordination abilities Excellent communication and interpersonal skills and comfort working independently while collaborating within a multidisciplinary team Proficiency with electronic medical records If you're passionate about improving the post-acute care experience and helping patients regain independence after hospitalization, we'd love to hear from you.
