Job Description
Join a growing Post-Acute Care team in Burlington, VT as a full-time Nurse Practitioner or Physician Assistant providing patient-centered care for individuals transitioning from acute hospitals to skilled nursing and rehabilitation facilities. Key responsibilities Provide facility-based rounding and management of acute and chronic medical conditions in post-acute care settings. Work collaboratively with interdisciplinary teams to support recovery, coordinate care, and minimize hospital readmissions. Perform clinical assessments, diagnostics, and treatment planning; adjust care plans based on patient progress. Communicate effectively with patients, families, nursing staff, and other providers; document care in the EMR. Practice with autonomy while leveraging available clinical and operational support.
