Job Description

We are seeking a dedicated and compassionate Primary Care Provider (MD, DO, NP, or PA) to join our dynamic team. The ideal candidate will provide comprehensive outpatient care, build long-term relationships with patients, and contribute to our mission of delivering high-quality healthcare in a personalized setting. This can be either a full-time or part-time position.
Responsibilities include:
Providing preventive, acute, and chronic care management for patients of all ages. Collaborate with a multidisciplinary team to ensure continuity of care. Maintain accurate and timely documentation in our EMR system. Participate in quality improvement initiatives and practice development; and foster a welcoming and respectful environment for patients and staff. Must be licensed (or eligible for licensure) in the state of Vermont. Have strong communication and interpersonal skills. Have a commitment to patient-centered care and community health.
