Vitalis Primary Care PLLC is recruiting a full-time Nurse Practitioner to provide high-quality adult primary care in Adult Family Homes and community-based settings in Washington State. This is an opportunity for an experienced clinician who values autonomy, strong clinical judgment, relationship-based care, and a lower-friction practice model focused on medically complex adults. We are looking for a clinically mature NP with a strong RN foundation and the judgment to work independently in the field. This role is ideal for an NP who is comfortable managing complexity, building trust with patients and caregivers, and practicing with both independence and accountability. Duties The Nurse Practitioner will provide longitudinal primary care, chronic disease management, post-hospital follow-up, urgent visits as needed, medication management, and care coordination for adult and geriatric patients residing in Adult Family Homes and similar residential settings. Deliver comprehensive primary care to adult and geriatric patients in community-based residential settings Perform assessments, diagnosis, treatment planning, and medication management Manage chronic illnesses, acute changes in condition, and transitional care needs Coordinate with caregivers, facility staff, specialists, pharmacies, labs, home health, and ancillary services Document clearly and efficiently in the EMR Support a proactive care model aimed at reducing avoidable hospitalizations and improving continuity of care Participate in practice development and quality improvement as the organization grows Requirements Active Washington State Nurse Practitioner license in good standing National board certification as a Nurse Practitioner DEA license and prescriptive authority, or ability to obtain promptly Minimum 5 years of RN experience prior to NP graduation Minimum 1 year of NP practice experience preferred Experience in primary care, geriatrics, home-based care, SNF/LTC, post-acute, or complex adult medicine strongly preferred Strong clinical assessment and independent decision-making skills Valid driver's license, reliable transportation, and willingness to travel locally.

Pay:

$120,000.00 - $170,000.00 per year
