This is a non-prescribing, field-based role focused on preventive care, patient education, closing care gaps, and collaborating with primary care providers. Key Responsibilities Conduct comprehensive in-home health assessments Review medical history and medications Perform physical exams, screenings, and point-of-care testing Identify and document diagnoses for care planning Communicate findings to primary care providers Educate patients on disease management, medications, and treatment compliance Address social determinants of health and connect members with appropriate resources Recognize and respond to urgent or emergent situations

Required Qualifications NP:

ANCC or AANP certification (Family, Adult, Geriatric, or Adult-Gerontology Primary Care)

PA:

NCCPA certification and active state licensure Active, unrestricted license (or ability to obtain by start date) Valid driver's license and reliable transportation Ability to work independently in a variety of home environments Preferred Qualifications 1+ years of clinical experience, preferably in family practice, geriatrics, primary care, or home health Experience with Medicare or geriatric populations EMR and technology proficiency Strong communication and patient education skills Ability to transport equipment up to 30 pounds and navigate stairs during home visits Highlights This role offers exceptional work-life balance, flexible scheduling, meaningful one-on-one patient interactions, a non-prescribing care model, and the opportunity to improve health outcomes by bringing preventive care directly into patients' homes. Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
