$30,000 Sign-On Bonus or $40,000 Student Loan Repayment Schedule Flexible schedules available from 24-40 hours per week

No call, weekends, or holidays About the Role Optum HouseCalls is seeking Nurse Practitioners and Physician Assistants to provide annual in-home health assessments for Medicare Advantage and other health plan members. This is a non-prescribing, field-based role focused on preventive care, patient education, closing care gaps, and collaborating with primary care providers. Key Responsibilities Conduct comprehensive in-home health assessments
Review medical history and medications
Perform physical exams, screenings, and point-of-care testing
Identify and document diagnoses for care planning
Communicate findings to primary care providers
Educate patients on disease management, medications, and treatment compliance
Address social determinants of health and connect members with appropriate resources
Recognize and respond to urgent or emergent situations
