Overview Join our growing senior care team as an Advanced Practice Clinician (ARNP or PA-C), providing comprehensive, patient-centered primary care in an outpatient clinic setting. This role focuses on preventive care, chronic disease management, Annual Wellness Visits, care coordination, and improving health outcomes for adult and senior patient populations. We are seeking a compassionate, relationship-driven provider who is passionate about helping patients maintain their health, independence, and quality of life through personalized, evidence-based care. Responsibilities Conduct thorough physical examinations and patient assessments to establish accurate diagnoses and develop personalized treatment plans. Prescribe medications, including controlled substances in accordance with DEA regulations, and manage ongoing medication therapies. Perform outpatient diagnostic and therapeutic procedures appropriate to a primary care setting, including wound management, skin biopsies, cryotherapy, joint injections, and other services consistent with training, certification, and clinic protocols. Collaborate closely with physicians, nurses, specialists, and other healthcare professionals to coordinate care and ensure seamless patient management across inpatient, outpatient, emergency, and telehealth settings. Utilize electronic health record (EHR) systems like eClinicalWorks for documentation, order entry, and clinical data management while maintaining HIPAA compliance. Provide education on disease management, preventive care, and wellness strategies tailored to diverse populations including pediatrics, geriatrics, behavioral health patients, and individuals with disabilities. Participate in Advanced Primary Care Management (APCM) initiatives by developing individualized care plans, addressing preventive care needs, and supporting patients in achieving their health goals. Requirements Valid certification as an Advanced Registered Nurse Practitioner (ARNP) or Certified Physician Assistant (PA-C), with current state licensure. Proficiency in EMR/EHR systems for efficient medical documentation and patient record management. Strong knowledge of physiology, anatomy, pharmacology (including CPT coding), and diagnostic evaluation techniques such as sonography or laboratory specimen processing. Excellent client communication skills with the ability to educate patients across age groups—including children and seniors—and manage complex cases involving behavioral health or memory care needs.
