This is a non-prescribing, field-based role focused on preventive care, patient education, care gap closure, and collaboration with primary care providers. Key Responsibilities Conduct comprehensive in-home health assessments Review medical history and medications Perform physical exams, screenings, and point-of-care testing Identify and document diagnoses to support care management and treatment planning Communicate findings to primary care providers Educate members on health conditions, medications, and treatment compliance Address social determinants of health and connect members with community resources Recognize and respond appropriately to urgent or emergent situations

Required Qualifications NP:

ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care

PA:

NCCPA certification and active state licensure Active, unrestricted license (or ability to obtain by start date) Valid driver's license and reliable transportation Ability to work independently in a variety of home environments Preferred Qualifications 1+ years of clinical experience in family practice, geriatrics, primary care, or home health Experience working with Medicare or geriatric populations EMR and technology proficiency Strong communication and patient education skills Ability to transport equipment up to 30 pounds and navigate stairs Fluency in a language other than English Highlights This role offers excellent work-life balance, flexible scheduling, meaningful one-on-one patient interactions, a non-prescribing care model, and the opportunity to improve health outcomes by bringing preventive care directly into patients' homes while earning additional incentives and bilingual pay opportunities.
