We're expanding our team that provides patient-centered primary care in skilled nursing facilities around Cumberland, WI. This full-time opportunity offers a flexible schedule with no overnight shifts or on-call requirements. You'll work alongside physicians and facility staff to deliver high-quality, evidence-based care to residents in a collaborative, supportive environment. What the schedule and role look like Full-time and part-time options with flexible scheduling. Typical shifts are six hours and rounds usually start between 7:00 a.m. and 10:00 a.m. No call or overnight responsibilities. Average caseload of about 15-20 patient visits per day. Daily rounding, managing both acute and chronic conditions, prescribing medications, and developing individualized treatment plans. Close collaboration with nursing staff and interdisciplinary teams to coordinate care. Responsibility for maintaining clinical and regulatory compliance. What we're looking for Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license.
NPs:
board certification through ANCC, AANPCB, PNCB, or AACN.
PAs:
NCCPA certification. Board certified or board eligible in FM, IM, Geriatrics, PM&R, or Emergency Medicine is preferred. Strong clinical judgment, excellent communication skills, and a patient-centered approach to care. Able to work independently while collaborating effectively within a multidisciplinary team.
