Job Overview We are seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to provide comprehensive primary care services within a School-Based Health Center (SBHC) operated by our Federally Qualified Health Center (FQHC). The provider will deliver age-appropriate care to students, promote wellness, and collaborate with school staff, families, and healthcare teams to improve student health outcomes.
Key Responsibilities:
Provide primary care services including assessment, diagnosis, and treatment of acute and chronic conditions Perform physical exams, well-child visits, and preventive health screenings Administer immunizations and manage medication therapies Promote health education and disease prevention among students Maintain accurate and timely electronic medical records (EMR) documentation Collaborate with school personnel, behavioral health providers, and community resources Participate in quality improvement initiatives and FQHC compliance requirements Ensure adherence to federal, state, and organizational regulationsQualifications:
Graduate of an accredited Nurse Practitioner or Physician Assistant program Current, unrestricted WV license Board certification (e.g., AANP, ANCC, NCCPA) DEA license (or eligibility) Experience in pediatrics, family medicine, or school-based care preferred Experience in an FQHC or underserved setting is a plus Certification in Basic Life Support (BLS) requiredWork Environment:
