Nurse Practitioner or Physician AssistantJOB PURPOSE
Provide direct primary care to patients, with emphasis on geriatric care, including treatments and medications. Locations of care include clinic, nursing home, hospice, assisted living and patients' homes.ESSENTIAL DUTIES AND RESPONSIBILITIES
Conduct comprehensive health assessments and physical examinations for patients. Complete recertifications as required by Medicare. Develop and implement individualized, long-term care plans. Manage chronic diseases and address acute medical issues. Prescribe medical care to patients, including treatments and medications. Collaborate with multidisciplinary teams to coordinate patient care. Monitor patient progress and adjust care plans as needed.COMPETENCIES

Master's degree inNursing Experience:
1-3 years nurse practitioner experience preferred Required licensure, certification, registration, or other requirements: Valid State of Wyoming authorization as a Nurse Practitioner. Prescriptive authority in the State of Wyoming. National certification in degree specialty. For certain specialized positions, a DEA registry license may be required. Valid CPR certification (cardiopulmonary resuscitation) Valid BLS certification (basic life support)MINIMUM QUALIFICATIONS

3 years as a Physician's Assistant Required licensure, certification, registration or other requirements: National and valid State of Wyoming certification as a Physician's Assistant Prescriptive authority in the State of Wyoming Licensure by the Wyoming Board of Medicine The level of this position will be determined based on the selected candidate's experience.DESIRED QUALIFICATIONS
