Job Description
Responsibilities:
- Provide personal care and assistance to patients in their homes
- Assist with activities of daily living, including bathing, dressing, and grooming
- Administer medication as directed by healthcare professionals
- Monitor and record vital signs, such as blood pressure and temperature
- Assist with mobility and transfers, using proper lifting techniques
- Prepare and serve meals according to dietary restrictions
- Perform light housekeeping duties, such as cleaning and laundry
- Provide companionship and emotional support to patients
- Follow care plans and communicate any changes in the patient's condition to the healthcare team
Qualifications:
- High school diploma or equivalent
- Certification as a Home Health Aide or completion of a recognized training program
- Experience in providing personal care and assistance to individuals with medical needs
- Knowledge of medical documentation procedures and practices
- Familiarity with hospice care principles and practices is a plus
- Strong communication skills and the ability to interact effectively with patients, families, and healthcare professionals
- Compassionate and patient-centered approach to caregiving
- Ability to prioritize tasks and manage time effectively
- Reliable transportation and a valid driver's license
Skills:
- Experience in providing patient care in an assisted living or home setting
- Knowledge of working with individuals with developmental disabilities is a plus
- Familiarity with memory care techniques for patients with cognitive impairments
- Ability to cook nutritious meals for patients based on dietary restrictions
- Proficiency in maintaining a clean and organized environment for patient safety
