Job Description

Responsibilities:
  • Provide personal care and assistance to patients in their homes
  • Assist with activities of daily living, including bathing, dressing, and grooming
  • Administer medication as directed by healthcare professionals
  • Monitor and record vital signs, such as blood pressure and temperature
  • Assist with mobility and transfers, using proper lifting techniques
  • Prepare and serve meals according to dietary restrictions
  • Perform light housekeeping duties, such as cleaning and laundry
  • Provide companionship and emotional support to patients
  • Follow care plans and communicate any changes in the patient's condition to the healthcare team
Qualifications:
  • High school diploma or equivalent
  • Certification as a Home Health Aide or completion of a recognized training program
  • Experience in providing personal care and assistance to individuals with medical needs
  • Knowledge of medical documentation procedures and practices
  • Familiarity with hospice care principles and practices is a plus
  • Strong communication skills and the ability to interact effectively with patients, families, and healthcare professionals
  • Compassionate and patient-centered approach to caregiving
  • Ability to prioritize tasks and manage time effectively
  • Reliable transportation and a valid driver's license
Skills:
  • Experience in providing patient care in an assisted living or home setting
  • Knowledge of working with individuals with developmental disabilities is a plus
  • Familiarity with memory care techniques for patients with cognitive impairments
  • Ability to cook nutritious meals for patients based on dietary restrictions
  • Proficiency in maintaining a clean and organized environment for patient safety
