Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts when necessary Monitor patient health status through patient observation, recording vital signs, and reporting changes promptly while maintaining accurate medical documentation within Electronic Health Record (EHR) systems Administer medications following home healthcare regulations and physician instructions, ensuring proper medication management and documentation Provide specialized care for clients with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies Support end-of-life care with compassion, dignity, and respect for patients' wishes while maintaining patient confidentiality in compliance with HIPAA standards Perform light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe living environment for clients Maintain effective communication with clients and their families to ensure personalized care plans are followed and needs are met Qualifications Valid Certified Nursing Assistant (CNA) license or equivalent certification required Proven experience working with geriatric patients, individuals with developmental disabilities, or in hospice or long-term care settings preferred Knowledge of medical terminology, catheter care, and vital signs measurement essential Experience operating EMR/EHR systems for accurate medical records management Strong understanding of home health care assistance protocols and home & community care regulations Certifications in First Aid, Basic Life Support (BLS), CPR required; additional certifications such as hospice care or dementia training are a plus Ability to lift heavy objects safely and perform patient transfers using appropriate techniques and equipment such as Hoyer lifts or patient transfer methods Excellent communication skills with a caring attitude; ability to work independently while following detailed care plans Join us if you're passionate about providing high-quality home health services that truly make a difference!
