Job Description

Elements of Elder Care is the best hospice in the greater Phoenix area! Our team is comprised of only the most compassionate and hard working hospice care workers! If you want to join a team that is caring, knowledgeable, and excellent at what they do, then we are the agency for you! Elements of Elder Care is looking for East Valley Hospice Aides to join our team!

The Hospice Aide will:

Provide patient care in the East Valley only - Mesa, Chandler, Apache Junction, Queen Creek, San Tan Valley areas with mileage reimbursement. Provide exceptional care according to the patient's personalized plan of care. Meet patient frequencies and tasks lists. Document patient visits in the Consolo Wellsky charting system. Drive to and provide care wherever the patient calls home. This includes group homes, facilities and private residences.

Requirements:

High school diploma or GED. Certified or licensed by the AZ Board of Nursing. CPR certified. Valid Driver license Current auto insurance. Elements of Elder Care is a work environment dedicated to diversity, equity and inclusion.

Pay:

$23.00 - $25.00 per hour
