Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment like Hoyer lifts. Monitor and document vital signs, medication administration, and changes in patient condition using Electronic Health Record (EHR) systems while maintaining strict adherence to HIPAA regulations to protect patient confidentiality. Provide specialized care for individuals with Alzheimer's disease, dementia, or other memory-related conditions through memory care techniques and personalized engagement strategies. Support patients with medical procedures including catheter care, wound dressing, and basic life support (BLS) or CPR when necessary. Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment that complies with home healthcare regulations. Communicate effectively with clients and their families to understand needs and provide emotional support while respecting cultural sensitivities and privacy standards. Follow protocols for hospice or end-of-life care when applicable, ensuring comfort and dignity for patients during sensitive times. Experience Prior experience working with seniors or individuals with disabilities is a plus! Experience working with geriatric patients or those requiring dementia or Alzheimer's care is a strong asset. Familiarity with EMR/EHR systems for accurate record keeping and documentation. Certification in first aid and CPR or BLS are a preferred. Proven ability to perform heavy lifting safely, including patient transfers using assistive devices like Hoyer lifts or patient transfer methods.
