0-49 Job Description This position works in conjunction with nursing staff, residents, families, and other facility departments to provide optimum individualized care for the residents of the SNF facility. Shifts include some weekends and holidays and may be required to float to other units as necessary. Good communication skills and the ability to work with others and be a team player is a must. Job Responsibilities Receive and give report on residents' conditions and ensure understanding of care to be provided. Make rounds on each resident and assess new residents and residents who are ill, have fallen, or sustained an injury. Document assessments according to facility policy. Notify nurses of changes in residents' conditions or current needs. Assist with direct patient care and respond promptly to call lights. Promote positive working relationships with departments, co-workers, residents, families, and physicians. Float to other units as necessary. Perform other duties as assigned.
