Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting, ensuring comfort and dignity. Safely transfer patients using patient transfer methods such as Hoyer lifts, gait belts, or wheelchairs, adhering to proper techniques. Monitor patient health by observing vital signs, reporting changes promptly, and documenting medical information accurately in Electronic Health Records (EHR) or Electronic Medical Records (EMR) systems. Provide specialized care for clients with dementia or Alzheimer's disease, including memory care strategies and behavioral management. Administer medications following home healthcare regulations and medical terminology guidelines while maintaining patient confidentiality under HIPAA standards. Support patients during end-of-life or hospice care with compassion, respecting their wishes and providing emotional support. Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry and meal preparation, ensuring compliance with health standards. Qualifications Valid Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification required. Proven experience working with geriatric patients or individuals with developmental disabilities is preferred. Experience operating mobility aids such as Hoyer lifts and familiarity with patient transfer methods. Knowledge of medical terminology, medication administration, catheter care, and vital signs monitoring. Strong understanding of home health care assistance regulations and documentation procedures. CPR and First Aid certification required; Basic Life Support (BLS) training preferred. Ability to work independently while communicating effectively with clients and team members.
