Job Description

Per Diem Position Rates:
$25-$40 per visit rates + mileage Overview We are seeking compassionate and dedicated Home Health Aides to provide essential support to individuals in their homes. The ideal candidate will possess a strong commitment to patient care and have experience in assisting with activities of daily living (ADLs). As a Home Health Aide, you will play a vital role in enhancing the quality of life for our clients, ensuring their comfort and safety while promoting independence. Responsibilities Assist clients with daily living activities such as bathing, dressing, grooming, and meal preparation. Provide companionship and emotional support to clients, fostering a positive environment. Monitor and document vital signs, reporting any changes in the client's condition to the healthcare team. Administer medications as prescribed and assist with medication management. Perform patient observation to identify any changes in health status or behavior. Assist with mobility and transfers using equipment such as Hoyer lifts when necessary. Maintain cleanliness in the client's living space, including laundry and light housekeeping tasks. Ensure compliance with HIPAA regulations and maintain confidentiality of client information. Support clients with Alzheimer's care, dementia care, and other memory-related conditions. Collaborate with family members and healthcare professionals to develop personalized care plans. Experience Previous experience in home health, hospice, senior care, or long-term care settings is preferred. Familiarity with EMR/EHR, primarily Kinnser, systems for medical documentation is advantageous. Knowledge of medical terminology and basic nursing skills such as CPR and catheter care is beneficial. Experience working with individuals who have developmental disabilities or require hospice care is a plus. Strong communication skills and the ability to work effectively with diverse populations are essential.
