Non-Exempt Position Summary The Home Health Certified Nursing Assistant (CNA) provides personal care and supportive services to patients in their homes under the direction and supervision of a licensed nurse and in accordance with the physician's plan of care. The CNA assists patients with activities of daily living, observes and reports changes in condition, and documents care accurately in the agency's electronic medical record system, including Data Soft Logic (DSL) EMR . Essential Duties and Responsibilities Provide hands-on personal care to patients in the home setting, including: Bathing Grooming Oral hygiene Dressing Toileting and incontinence care Ambulation and transfers Range of motion exercises as assigned Assist patients with light housekeeping related to patient care. Assist with meal preparation and feeding, as permitted by the care plan. Take and record vital signs as assigned. Observe patient condition and promptly report changes to the supervising nurse. Maintain a safe, clean, and comfortable environment for the patient. Follow infection control, safety, and confidentiality policies at all times. Document all patient care, observations, and visits accurately and timely in Data Soft Logic EMR . Review assigned tasks, visit notes, schedules, and care plans in the EMR system. Communicate effectively with nurses, therapists, physicians' offices, patients, and family members as needed. Comply with agency policies, state and federal regulations, and accreditation standards. Qualifications Current and active Certified Nursing Assistant (CNA) certification in the state of employment. Current BLS/CPR certification . Minimum 1 year of healthcare experience preferred; home health experience preferred. Valid driver's license, reliable transportation, and current auto insurance. Ability to travel to patient homes within assigned territory. Strong interpersonal, communication, and observation skills. Ability to maintain patient confidentiality and professionalism. EMR / Technical Requirements Must be able to learn and effectively use Data Soft Logic (DSL) EMR system for: Reviewing patient demographics and schedules Accessing care plans and visit assignments Completing visit documentation Reporting patient observations Maintaining timely and accurate electronic records Prior experience with Data Soft Logic EMR is preferred, but training may be provided. Basic computer, tablet, and mobile documentation skills required. Physical Requirements Ability to stand, walk, bend, stoop, kneel, and lift or transfer patients with assistance as needed. Ability to lift up to 25-50 pounds consistent with patient care duties. Ability to travel between patient homes. Work Environment Services are provided in patients' homes. Exposure to varying home environments and conditions. Frequent local travel required. Knowledge, Skills, and Abilities Knowledge of personal care principles and basic patient safety practices. Ability to follow written and verbal instructions. Ability to recognize and report changes in patient condition. Ability to use good judgment and maintain compassionate patient-centered care. Ability to document clearly and accurately in Data Soft Logic EMR . Preferred Qualifications Home health or hospice experience. Experience documenting in Data Soft Logic EMR . Bilingual skills are a plus.

$35.00 - $40.00 per hour
