About Us:
Pro Angels Hospice is a compassionate, patient-centered hospice agency dedicated to providing exceptional end-of-life care. Owned and operated by experienced healthcare professionals, our mission is to support patients and their families with dignity, respect, and excellence in care.
Position Summary:
We are currently seeking a certified, per diem Hospice Aide (HA) to join our team. As we continue to grow and admit new patients, we are looking for dependable, caring individuals who are passionate about providing comfort and support to patients and their families. Hospice Aides are assigned patients at the time of admission and provide care two to three times a week per patient. Assignments rotate based on patient admissions. You will work closely with our Director of Patient Care Services (DPCS) and other clinical team members.
Key Responsibilities:
Provide personal care and companionship to hospice patients in their homes. Complete timely and accurate documentation of each visit using our EMR system (DSL). Report any changes in patient condition to the Director of Patient Care Services. Deliver care with compassion, professionalism, and respect. Collaborate with the interdisciplinary team to ensure high-quality, individualized care.
Qualifications:
Current certification as a Home Health Aide (CHHA) or Certified Nursing Assistant (CNA) in California. CPR certification required. Previous hospice experience is required. Familiarity with DSL is a plus. Strong communication and interpersonal skills. Reliable transportation and ability to travel to patient homes. Compassionate, dependable, and professional demeanor.
