Job Overview We are seeking a dedicated and compassionate Per Diem Certified Nursing Assistant (CNA) / Certified Home Health Aide (CHHA) to join our dynamic healthcare team. This role offers flexible per diem shifts, allowing you to provide vital patient care across various settings, including assisted living facilities, nursing homes, and home health environments. As a key member of our care team, you will deliver personalized support to individuals with diverse needs, including those with developmental disabilities, dementia, Alzheimer's disease, and our energy and commitment will directly enhance the quality of life for our clients while fostering a positive and supportive care environment. This a great side income opportunity with a new agency. Home Health experience preferred, but willing to train! Responsibilities Perform patient observation and monitor vital signs to assess health status regularly. Assist with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility assistance. Document medical records accurately using Electronic Health Record (EHR) systems while maintaining HIPAA compliance. Support patients with specialized care with activities of daily living. Operate medical equipment such as Hoyer lifts for safe patient transfers Engage in home & community care activities including meal preparation, cleaning, laundry, and light housekeeping to promote a safe living environment. Collaborate with nursing staff to update medical documentation and ensure seamless communication regarding patient progress. Visit and conduct services in the patients home Ability to be professional, patient and empathetic Requirements Valid certification as a CNA or CHHA with active state license or registration. Prior experience in nursing homes, assisted living facilities, or home health environments preferred, BUT

WILLING TO TRAIN!

Strong knowledge of medical terminology, patient care techniques, and safety protocols including CPR certification. Experience working with patients with dementia, Alzheimer's disease, or in hospice care is highly desirable. Familiarity with EMR/EHR systems for accurate documentation of patient information. Ability to work independently and adapt quickly to changing patient needs in a variety of settings. Ability to travel and treat patients

Pay:

$20.00 - $25.00 per hour Expected hours: 4.0 - 10.0 per week
