Job Summary We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) / Caregiver to join our team in providing exceptional care to individuals across various care settings. In this vital role, you will support residents and patients by assisting with daily activities, monitoring health status, and ensuring a safe, comfortable environment. Your energy and positive attitude will make a meaningful difference in the lives of those you serve, helping them maintain independence and dignity while receiving personalized care. Responsibilities Assist residents and patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and observe patient conditions, recording vital signs such as blood pressure, pulse, temperature, and respiration accurately in electronic health record (EHR) systems Support medication administration under supervision, ensuring proper documentation following home healthcare regulations and HIPAA guidelines Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies Assist with medical procedures such as catheter care, wound dressing changes, and hospice/end-of-life care needs Support safe transfer techniques using equipment like Hoyer lifts or other assistive devices while adhering to safety protocols for heavy lifting and patient handling Maintain a clean environment by assisting with laundry, cleaning duties, meal preparation, and ensuring infection control standards are met Requirements Valid certification as a Nursing Assistant or Caregiver with active state licensure or registration Proven experience working with geriatric patients in assisted living facilities, nursing homes, or home healthcare settings Familiarity with EMR/EHR systems for accurate medical documentation and record keeping Knowledge of medical terminology relevant to patient care and medication administration procedures Experience working with individuals with developmental disabilities or disabilities requiring specialized attention Ability to perform CPR and basic life support (BLS), with current certifications preferred Strong understanding of home health regulations, HIPAA compliance, and safety protocols for hospice and end-of-life care Excellent patient observation skills to detect changes in condition or behavior promptly Physical ability to perform heavy lifting tasks safely and assist with transfers using appropriate equipment such as Hoyer lifts Compassionate caregiving attitude combined with excellent communication skills to work effectively within a team environment Join us in making a positive impact through dedicated caregiving!
