In this vital role, you will deliver personalized patient care, assist with daily living activities, and support individuals with diverse healthcare needs. Your energetic approach and attention to detail will ensure each client receives respectful, professional, and comprehensive support in their home environment. Responsibilities Provide direct patient care by assisting with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility assistance. Monitor patient health through observation of vital signs, medication administration, and reporting any changes or concerns promptly. Support clients with specialized needs such as Alzheimer's care, dementia care, hospice care, and end-of-life support. Utilize electronic health record (EHR) systems to accurately document medical information, patient progress, and care provided. Assist with transferring patients safely using patient transfer methods such as Hoyer lifts or other equipment while adhering to safety protocols. Maintain patient confidentiality in compliance with HIPAA regulations and ensure all medical documentation is accurate and up-to-date. Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients. Communicate effectively with clients and their families to provide updates on care plans and address any concerns or questions. Follow home healthcare regulations and standards to deliver high-quality care that meets legal and safety requirements. Qualifications Valid CNA certification with current licensure in the state of employment. Proven experience working in home healthcare settings or assisted living facilities preferred; experience in nursing homes or geriatrics is highly valued. Knowledge of medical terminology, catheter care, medication administration, and vital signs monitoring. Experience working with individuals with developmental disabilities or disabilities is a plus; familiarity with dementia or Alzheimer's care is desirable. Ability to operate EMR/EHR systems efficiently for documentation purposes. Strong understanding of home health care assistance, patient transfer methods, and safety procedures including CPR and basic life support (BLS). Excellent communication skills to foster positive relationships with clients, families, and healthcare teams. Physical ability to perform heavy lifting when necessary and assist with mobility tasks safely. Knowledge of home & community care regulations along with HIPAA compliance standards.
