Our clients have planned for their retirement and end of life care in order to stay in their homes. Most of our clients have established that they want to reside in their home as long as possible, preferably until the end of life. Our agency promotes that seniors rehabilitate quicker and better in the privacy of their own home. We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) with specialized experience in rehabilitation care, memory care, and Parkinson's disease management. In this vital role, you will provide unparalleled care, exceptional patient-centered attention to individuals with complex needs, including those living with Alzheimers, dementia, neurological conditions, and post-acute recovery requirements. Responsibilities Deliver direct patient care focusing on rehabilitation, memory support, and Parkinson's disease management, ensuring comfort and safety at all times Assist residents with activities of daily living such as bathing, dressing, grooming, mobility assistance using equipment like Hoyer lifts and transfer methods Monitor vital signs, medication administration, and perform basic clinical tasks following established protocols to support ongoing health assessments Maintain accurate medical documentation within Electronic Health Records (EHR) systems while respecting patient confidentiality under HIPAA guidelines Support cognitive engagement activities tailored for individuals with dementia or neurological conditions to enhance quality of life Collaborate closely with nursing staff and interdisciplinary teams to coordinate care plans and communicate resident progress effectivel Qualifications Valid CNA certification with current licensure in the applicable state Proven experience working with individuals requiring memory care, specifically dementia and Alzheimers care, neurological conditions such as Parkinson's disease Familiarity with electronic medical records (EMR/EHR systems) including Cerner or eClinicalWorks preferred Demonstrated knowledge of vital signs measurement, medication administration, and basic life support (BLS) certification Experience working in long-term care facilities, one-on-one care personalized care, assisted living communities, or post-acute settings for recovery clients Strong communication skills for caregiver interactions and patient service excellence Ability to perform heavy lifting safely using proper lifting practices and manual handling techniques. Caregiver should be able to assist with ADL's, perform lifts, transfers, fully ambulate and provide showers for non-mobile clients. Our company has been growing strong since 2018.
