Patient Transport/CNA Hybrid Park Forest Care Center - 2.5 Westminster, CO Job Details Full-time $22 - $28 an hour 1 hour ago Benefits Paid time off Qualifications Vital signs Load securement CNA Bathing assistance Patient observation Full Job Description Patient Transportation Driver / Certified Nursing Assistant (CNA) Hybrid Park Forest Care Center is seeking a dependable and safety-focused Patient Transportation Driver/CNA. This position is responsible for safely transporting residents to medical appointments and other approved destinations. When transportation is not scheduled, the employee will work on the nursing floor and perform regular CNA duties. Transportation Responsibilities Safely transport residents to and from medical appointments, dialysis, hospital visits, and other approved destinations. Properly secure each wheelchair to the vehicle using all required tie-down points. Ensure every resident is secured with the appropriate occupant restraint or seat belt before the vehicle moves. Complete a safety check before every trip to confirm that wheelchairs, mobility devices, residents, and equipment are properly secured. Operate the facility vehicle safely and follow all traffic laws, facility policies, and transportation procedures. Assist residents with entering and exiting the vehicle while using appropriate transfer techniques and equipment. Remain attentive to residents' physical and emotional condition throughout transport. Maintain communication with nursing staff regarding appointment times, delays, resident needs, and changes in condition. Immediately report all accidents, falls, injuries, vehicle concerns, or unusual events to the NHA, DON, or designated facility leader. Contact emergency services when necessary and remain with the resident until assistance arrives. Complete required transportation logs, mileage records, appointment documentation, and incident reports accurately and timely. Perform and document pre-trip and post-trip vehicle inspections. Maintain the cleanliness, organization, fuel level, and general readiness of the facility vehicle. Promptly report mechanical problems, damaged safety equipment, or concerns involving wheelchair securement systems. Protect residents' privacy and confidential health information during transportation and at appointments. Treat residents, families, facility staff, and medical office personnel with professionalism and respect. CNA Floor Responsibilities When not transporting residents, the employee will be assigned to the nursing floor and will: Provide direct care in accordance with each resident's care plan. Assist residents with bathing, dressing, grooming, toileting, eating, transfers, repositioning, and mobility. Answer call lights promptly and respond appropriately to resident needs. Obtain and document vital signs, weights, intake and output, and other required information. Observe residents and immediately report changes in condition to the licensed nurse.
