Responsibilities Deliver personalized patient care by assisting with activities of daily living such as bathing, dressing, feeding, and mobility Monitor and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates Support patients with medical procedures under supervision, including catheter care and basic wound management Maintain strict adherence to HIPAA regulations and ensure patient confidentiality at all times Document patient information precisely using Electronic Health Record (EHR) systems and medical documentation protocols Assist with patient transfers utilizing safe lifting practices and equipment such as Hoyer lifts or transfer belts Collaborate effectively with nursing staff, physicians, therapists, and family members to coordinate comprehensive care plans Skills Proven experience working with diverse patient populations in settings such as long-term care facilities, hospitals, assisted living, or home care environments Strong knowledge of medical terminology, anatomy, physiology, and aseptic techniques necessary for safe patient care Familiarity with EMR/EHR systems like Epic or Cerner for accurate documentation and data management Ability to perform vital signs measurement accurately and recognize signs of patient distress or deterioration Experience working with individuals with developmental disabilities, dementia (including Alzheimer's and memory care), or behavioral health needs Skilled in safe manual handling techniques and use of assistive devices such as Hoyer lifts or patient transfer methods Knowledge of infection control standards and procedures for infectious disease care and hospital environments including ICU or trauma center experience Join us to be part of a compassionate team committed to excellence in patient-centered care.
