Responsibilities Assist residents or patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs, patient behavior, and overall condition; promptly report changes to healthcare team Support medication administration following prescribed care plans and HIPAA confidentiality standards Facilitate safe patient transfers using equipment such as Hoyer lifts and patient transfer methods Prepare nutritious meals and assist with meal planning to meet individual dietary needs Maintain a clean and safe environment by performing regular cleaning duties and adhering to infection control protocols Document care activities accurately in compliance with healthcare regulations and resident rights policies Provide emotional support and engage residents in meaningful activities to promote mental well-being Qualifications Prior experience working in group homes, nursing homes, assisted living, or home health environments is highly preferred Knowledge of dementia care, Alzheimer's care, memory care, and hospice care practices Familiarity with patient observation techniques, vital signs measurement, medication administration, and manual handling skills Certification in CPR and First Aid is required; additional certifications such as HIPAA training are a plus Experience supporting individuals with autism or neurodevelopmental disabilities is advantageous Ability to lift heavy objects safely and perform physical tasks involved in patient transfer methods Strong communication skills for effective client interaction and teamwork Must demonstrate compassion, patience, attention to detail, and adherence to resident's rights policies Join us to provide compassionate care that truly makes an impact!
