at The Lorelton in Wilmington, Delaware, United States Job Description C.N.A (P/T and PRN) Promotes a lifestyle approach to actively engage residents in personal care activities. Understand each resident's capabilities and preferences in personal care activities; gets to know family's preferences for continued involvement in the resident's personal care. Actively engages residents and promoted self-care while assisting with personal care as specified in the resident service plan including: personal hygiene, bathing, toileting, dressing, and undressing, night time care, dining, ambulating and transfers. Assists with behavioral and cognition including: orientation, communication, challenging behaviors, and support. Promotes a lifestyle approach to actively engage residents in productive activities. Understand each resident's ability and preference in regards to activities such as household work, volunteering, personal hobbies, and community work etc... look for ways to engage the residents in activities they would prefer to do. Encourage residents when appropriate to participate in housekeeping and laundry tasks. Assist with the performance of cleaning resident rooms, bathrooms, vacuuming, mopping, and laundry. Encourage residents to participate in stocking pantry and other supplies to the extent the resident desires. Perform specified tasks such as taking inventory, retrieving, and restocking supplies. Participate in meal service; performs and helps monitor specified tasks such as setting, serving, and cleaning the dining area. Promote a lifestyle approach to engage residents in leisure activities. Understand what the resident's preferred leisure activities are such as: sitting quietly, socializing, hobbies, games, puzzles, TV, etc… look for ways to engage residents and involve their family members for quality time. Lead scheduled group activities; assist the Activities Director or designee in planning group activities. Assist residents with one on one and solo leisure activities. Assist DON to monitor and address residents' health status. Take vital signs, observe for symptoms, encourage compliance with treatment plan, observe nutritional status, etc… Recognizes and responds to emergency situations Assists residents with medications using the AWSAM program as permitted by the State of Delaware Ensures atmosphere of teamwork and open communication. Maintains a positive work environment by conveying respect, encouraging teamwork, and demonstrating willingness to pitch in and help with other areas of service as needed. Documents changes/exceptions and recommends changes to the service plan for each resident. Acknowledges resident and family concerns, initiates timely action, and reports problem/resolution to the DON. Communicates any issues, concerns, ideas, or policy questions to the DON. Recruits replacement staff when not able to work according to the schedule. Participates in orientation and training of new certified nursing assistants. Participates in staff meetings and in-services. Complies with building processes Is willing to work scheduled hours, which include weekends and holidays. Conducts round as assigned. Complies with all safety, sanitation, and emergency policies and procedures; responds to/reports all hazardous conditions and accidents. Other job functions Attends all required in-services and staff meetings Participates in performance appraisals and pursues individual development with direction from supervisor.
