This role involves supporting patients with a variety of healthcare needs, including assistance with daily activities, medication management, and health monitoring. Ideal candidates will have experience working with diverse populations, including seniors, individuals with developmental disabilities, dementia, or those requiring hospice or memory care. The position offers opportunities to make a meaningful difference in patients' lives while working in a supportive environment that emphasizes professionalism and compliance with healthcare regulations. Responsibilities Provide personalized patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support Monitor and record vital signs accurately and report changes to healthcare professionals Assist with medication administration following prescribed protocols Support patients with Alzheimer's, dementia, hospice, or memory care needs using specialized techniques Operate Electronic Health Records (EHR/EHR systems) to document patient interactions and medical information securely in compliance with HIPAA regulations Assist with medical procedures such as catheter care and Hoyer lift transfers Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and preparation Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment for patients Document medical records accurately and ensure compliance with healthcare standards Collaborate effectively with healthcare teams to coordinate patient care plans in assisted living, nursing homes, or home settings Qualifications Proven experience in home health, long-term care, assisted living, or nursing home environments Knowledge of medical terminology and caregiving best practices for dementia, Alzheimer's, hospice, and memory care patients Certification as a CNA or Home Health Aide is required; CPR certification is preferred Ability to work independently while following detailed care plans and instructions from healthcare professionals Strong communication skills in Spanish; proficiency in English is a plus Experience working with people with disabilities or developmental challenges is desirable Familiarity with EMR/EHR systems for documentation purposes Compassionate attitude with excellent patient observation skills and attention to detail Ability to handle physical tasks such as lifting or transferring patients safely using equipment like Hoyer lifts Must be reliable, punctual, and able to work flexible hours in home or community settings This position is ideal for caring individuals committed to improving the quality of life for their patients through dedicated service.
