In this vital role, you will provide exceptional in-home and community-based care to diverse patient populations, including seniors, individuals with disabilities, and those requiring specialized memory or hospice care. Duties Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature Administer medications accurately following prescribed schedules and medical documentation protocols Support patients with medical procedures like catheter care and wound management while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards Utilize electronic health record (EHR) systems to document patient progress, care plans, and medical records efficiently Employ patient transfer methods safely using equipment such as Hoyer lifts or transfer belts to prevent injury Provide emotional support and companionship tailored to patients with dementia, Alzheimer's disease, or end-of-life needs Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment for clients Communicate effectively with clients and their families about care plans and progress to foster trust and transparency Follow all safety protocols including first aid, CPR certification requirements, and emergency response procedures Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly preferred Demonstrated knowledge of medical terminology, medication administration, catheter care, and vital signs monitoring Experience working with individuals with developmental disabilities or dementia is advantageous Familiarity with EMR/EHR systems for accurate documentation of patient information Proven ability to handle heavy lifting safely using appropriate patient transfer techniques such as Hoyer lifts or transfer belts Background in geriatrics or hospice care is a plus but not required; a strong caregiving background is essential Certification in first aid and CPR is mandatory; additional certifications in home health or hospice are beneficial Join us in delivering compassionate care that transforms lives!
