Overview We are seeking compassionate and dedicated Certified Nursing Assistants (CNAs) and Home Health Aides (HHAs) to provide exceptional patient care across various settings, including assisted living facilities, nursing homes, and home health environments. The ideal candidates will have experience working with diverse populations, including seniors, individuals with developmental disabilities, and those requiring memory or dementia care. This role offers an opportunity to make a meaningful difference in patients' lives by delivering personalized care with professionalism and empathy. Applicants should possess a strong understanding of medical terminology, patient observation, and documentation protocols, including familiarity with EMR/EHR systems. All positions are paid and require a commitment to maintaining HIPAA compliance and patient confidentiality. Duties Provide direct patient care by assisting with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility assistance. Monitor and observe patients' conditions, reporting any changes or concerns promptly to healthcare professionals. Administer medications and perform treatments such as catheter care, Hoyer lift transfers, and vital sign measurements following established protocols. Document patient information accurately using EMR/EHR systems while maintaining HIPAA compliance. Support patients with meal preparation and feeding as needed, ensuring nutritional needs are met. Assist with light housekeeping tasks such as laundry and cleaning to maintain a safe and sanitary environment. Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques. Support hospice or palliative care patients with compassionate attention to comfort and dignity. Utilize medical documentation skills to record patient progress and communicate effectively within the healthcare team. Qualifications
