Requirements:
Valid Driver's License Professional License (CNA) Free of Communicable Disease Statement
  • Dated/Signed within last 6 months CPR Card (Only American Heart Association, American Red Cross, OR American Safety and Health Institute HIV Certificate
  • 1 hour Alzheimer's/Dementia
  • 2 or more hours approved by DOEA only Auto registration and auto insurance if providing transportation Previous Employment/Contract history for the last five years Professional Liability Insurance
