8:30am-5pm and every other weekend.
Location:
Field based covering Volusia with some occasional coverage into Flagler County. Mileage reimbursement of 76 cents per mile. Provides personal care to patients according to the assignment details and care plan. Documents patient care and observations timely and appropriately. Participates in end-of-life care for patients and families. Informs the registered nurse of any changes in the patient's condition. Follows the schedule of care, including making several visits per day with travel. Assists patients with activities of daily living, such as bathing, dressing, and grooming. Monitors and records vital signs, including temperature, pulse, and respiration. Supports patients with mobility and exercise as prescribed. Provides emotional support and companionship to patients. Maintains a clean and safe environment for patients. Communicates effectively with patients, families, and healthcare team members. Other duties as assigned.
