Job Description
Job Overview Join our compassionate healthcare team as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA), where your dedication to patient care makes a meaningful difference every day. In this vital role, you will provide essential support to patients across various settings, including long-term care facilities, hospitals, assisted living communities, and home care environments. Duties Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and manual handling techniques. Monitor and record vital signs, including blood pressure, pulse, temperature, and respiration rates, ensuring accurate documentation in electronic health record (EHR) systems. Provide medication reminders and assist with medication administration under supervision, adhering strictly to HIPAA regulations and safety protocols. Support patients with specialized needs such as dementia or Alzheimer's care, memory care, behavioral health issues, or those with developmental disabilities. Maintain a safe environment by practicing aseptic techniques during procedures like catheter care and phlebotomy when applicable. Collaborate with nursing staff and other healthcare professionals in acute care settings such as ICU, emergency medicine, trauma centers (Level I & II), PICU (Pediatric Intensive Care Unit), or surgical units to deliver comprehensive patient service. Assist with patient transfers using equipment like Hoyer lifts and ensure safe manual handling practices during movement or repositioning. Document all patient interactions accurately within EMR/EHR systems while respecting confidentiality and HIPAA standards. Requirements Valid CNA certification or HHA certification with current state licensing requirements. Prior experience working in diverse healthcare environments such as hospitals (including ICU, trauma centers), long-term care facilities, assisted living communities, or home health settings. Demonstrated knowledge of medical terminology, anatomy, physiology, and infection control practices. Experience with EMR/EHR systems such as Epic, Cerner, eClinicalWorks or similar platforms for medical documentation management. Ability to perform heavy lifting and physical tasks safely while adhering to safe lifting practices; capable of standing, walking long periods, and assisting patients with mobility needs. Strong communication skills to effectively collaborate with multidisciplinary teams and engage with patients compassionately. Knowledge of HIPAA regulations and commitment to maintaining patient confidentiality at all times. Additional desirable skills include experience in geriatrics or pediatric care, hospice or palliative care experience, working with individuals with disabilities or behavioral health needs, medication administration expertise, and familiarity with medical-surgical procedures or outpatient clinics. A Perfect Choice Home Care is looking for caregivers, home health aides and CNA'S for our elderly clients in the Bradenton, Sarasota and surrounding areas! Responsibilities include assisting with ADLs, companionship, light cleaning, meal prep, transportation, and medication reminders.
